Participants will support each other to learn how to better manage various aspects of their life with chronic liver disease and learn about a range of topics.
The Chronic Disease Self-Management model has consistently shown improvements in the confidence and health literacy of participants which by extension shows improvements health outcomes.
Each week an expert and facilitators will coach participants to improve self-efficacy and empower them to take control of their own health journey. Participants will receive up to date information from various domains as well as education around goal setting and putting these goals into action!
The program is open to anyone with lived experience of liver disease/a liver condition.
Session 1 – Introductions and Navigating the Health System: Saturday 20th October,10am – 12pm
Session 2 – Diet and Nutrition: Saturday 27th October, 10am –12pm
Session 3 – Exercise and Fitness: Saturday 3rd November, 10am – 12pm
Session 4 – Thoughts, Emotions, and Sleep Hygiene: Saturday 10th November, 10am–12pm
Session 5 – Medication management and Alcohol and other drugs: Saturday 17th November, 10am–12pm
